(1) Please use a separate registration form for each student.

(2) Upon completion of this form, you will be sent an invoice. To secure your place in the class, you must send payment of the Membership Fee (when required),  plus either the (a) full OR (b) minimum-allowed payment stated on the invoice WITHIN TWO WEEKS of us sending you the invoice.  If we do not receive your payment within this time period, your place in the class is not confirmed.
